Pueblo StepUp has two programs-Healthy Communities and the Medical Assistance Site.

Healthy Communities is a comprehensive community-based outreach program designed to assist families, children and pregnant women to find appropriate services. Through this program, family health coordinators connect families with resources to obtain treatment for medical, dental, vision, mental health, and developmental problems. All families eligible for CHP+ or Medicaid are encouraged to use these services.
The Healthy Communities program is for children and young age 20 and under and pregnant women. It helps to provide a medical home that helps children get the medical and non-medical services they need.
Family health coordinators also assist with prenatal care and education, arranging for client transportation to appointments where available, and linking clients to important community services. Healthy Communities strives to provide a comprehensive resource plan to clients to help them keep their families healthy for years to come.

Pueblo StepUp’s MA Site is a Medical Assistance Site certified through Colorado’s Department of Healthcare Policy and Financing. This authorizes Pueblo StepUp to accept and determine eligibility for State Medicaid Programs such as Family Medicaid, Child Health Plan Plus (CHP+), Adult Medicaid and Long Term Care to qualifying individuals in Pueblo and Pueblo County. Enrollment is offered at our main office. Pueblo StepUp also offers application processing to Pueblo Community Health Center, InnovAge, and Valley-Wide Health Systems, who has various locations in the southern region of the state.
Benefits of using an MA Site include:

•Provides an alternative source of assistance in completing the application process
•Timely eligibility determinations through CBMS
•Provides community resources and referrals for various medical programs.

Some applicants may receive immediate temporary health coverage the same day the application is received if the applicant qualifies for Presumptive Eligibility.
